Most health insurance plans will cover “Home Medical Equipment,” or “Durable Medical Equipment” including compression garments that are over 20mmHg of compression. Plans may vary, so check to see how many pairs of compression stockings per year you are eligible to receive.

You can bill your insurance for reimbursement by submitting the information below:

  1. A copy of your MartinHealthLine.com receipt of order and payment.
  2. A prescription from your physician for “over the counter” compression stockings, or an “over the counter” compression sleeve that includes your diagnosis. The prescription will not be valid if dated after placing your order, nor if it is dated over one year prior to your order.
  3. A letter of medical necessity from your physician (only needed with some insurance plans.)
  4. A letter stating your name, address, birthdate, policy number, the products purchased, along with appropriate medical billing codes (HCPC code). Billing codes are available upon request if you are unable to obtain them from your insurance company.
